Ensign College Health Plan Coverage

WHAT YOU NEED TO KNOW

The Ensign College Health Plan is administered by Deseret Mutual Benefit Administrators (DMBA). Once you have paid your health plan premium, the Cashier’s Office will confirm your coverage with DMBA. You are not considered “insured” by the health plan until you have paid your premium.

TYPES OF STUDENT COVERAGE AND COSTS

Please note that Student Health Plan premiums vary by semester. The premiums listed below apply only to the semester indicated.

This book is provided by Deseret Mutual Benefit Administrators and gives a detailed summary of what this health plan provides you. It also lists approved health centers and hospitals in case of an emergency.

PRINTING YOUR DMBA HEALTH CARD

Get your login.

To print your temporary DMBA ID card, you will need your Deseret Mutual Identification (DMID). You will need to enter this number as your LoginID. If you have been covered by the Student Health Plan before, you will already be familiar with this number. If you are new to the plan, or if you have forgotten this number, please contact DMBA .

Request your Student Health Plan card.

Once you have your ID number, head to www.dmba.com to create a password and log in. Click on the “Medical/Dental/Rx” tab at the top of the page, then select “ID Card” on the left-hand side.

From this drop-down list you can view and print your temporary ID card so you can fill prescriptions or pay for a doctor’s appointment. You can also have your temporary card faxed or emailed to you and request a permanent ID card be mailed to you.

Watch your mailbox.

Your health plan card will not be mailed from DMBA. You’ll receive an envelope from “VRx” containing your ID card, so please make sure you watch for it! Many students throw away the VRx mail without realizing what it contains.
