WHAT YOU NEED TO KNOW
The Ensign College Health Plan is administered by Deseret Mutual Benefit Administrators (DMBA). Once you have paid your health plan premium, the Cashier’s Office will confirm your coverage with DMBA. You are not considered “insured” by the health plan until you have paid your premium.
Get the health plan details by downloading the Student Health Plan Booklet. It also lists approved health centers and hospitals in case of an emergency.
TYPES OF STUDENT COVERAGE AND COSTS
Please note that Student Health Plan premiums vary by semester. The premiums listed below apply only to the following semesters: Fall 2025, Winter 2026, Spring 2026.

* Extended coverage is available for part-time students, students on semester break, and alumni (up to four months after graduation). Cost is per month. To request extended coverage, please download and fill out the Extended Coverage Request form and return it to the Cashier's Office (cashier@ensign.edu).
Get Your DMBA ID Number
You can find your DMBA ID# by contacting the Cashier's Office on the 2nd floor:

You can also contact DMBA directly at 801-578-5600. If you call DMBA, here are the prompts:
- Press 1 for English or Press 2 for Spanish
- Then press 1 on the next choice
- The computer will ask for your DMBA ID #. If you don't have your DMBA ID#, simply press # and it will send you through to a customer service representative who will be able to assist you. Please have your Ensign student ID# ready to help them locate your records.
Access/Request Your ID Card
- Go to dmba.com
- Click on "View ID Card" at the bottom of the page.
- In the new screen that opens, look at the bottom of the login section and select "Create new user login.”
- Follow the instructions to set up your secure access to the DMBA website. If it gives you an error telling you the ID doesn't match, call DMBA at 801-578-5600. They will need to help you set up online portal access.
- After you create your portal access, you can take a picture, print, or request a card be emailed or mailed to you.
