Exception to Admission Policy —To request an appeal to your decision or submit any additional requests to receive an exception to your application

Documentation of Disability Form - Students are required to have documentation that includes a recent, formal diagnosis from a healthcare provider licensed to diagnose their disability. This is a blank form that can be given to the student’s healthcare provider.

Release of Information Form - This form is utilized to request that disability related information regarding the student be released from the college or from the student’s healthcare provider to the college.

Note: You must have an active Ensign College account and be logged into your Ensign College email in a separate tab to access these forms. (They will not work if you are logged in to another email).

Medical Provider Form For Medical Reduced Course Load

Academic Suspension Review —If there are extenuating circumstances to an academic suspension status.

Change of Personal Information —To change name, date of birth, and identifying government numbers on Ensign’s official records. Once you have completed your change of personal information form, please upload it securely through OneDrive .

Diploma Reprint Request —To request an additional electronic and paper copy of your credential.

Exception to Academic Policy —Late graduation application, other extenuating circumstances to academic records.

Leave of Absence —In cases of military or mission service, students may be eligible for a leave of absence.

Permission to Enroll —Only available during registration periods.

Late Add —Add classes after the semester/block begins

Studio E Permission Form —In order to take the Studio E course and receive Prior Learning Credit.

Transfer Course Evaluation —For use AFTER initial transfer evaluation is complete. Request a one-to-one equivalency for a transfer course which was not accepted.
